Transthoracic Doppler echocardiography – noninvasive diagnostic window for coronary flow reserve assessment
نویسنده
چکیده
This review focuses on transthoracic Doppler echocardiography as noninvasive method used to assess coronary flow reserve (CFR) in a wide spectrum of clinical settings. Transthoracic Doppler echocardiography is rapidly gaining appreciation as popular tool to measure CFR both in stenosed and normal epicardial coronary arteries (predominantly in left anterior descending coronary artery). Post-stenotic CFR measurement is helpful in: functional assessment of moderate stenosis, detection of significant or critical stenosis, monitoring of restenosis after revascularization. In the absence of stenosis in the epicardial coronary artery, decreased CFR enable to detect impaired microvascular vasodilatation in: reperfused myocardial infarct, arterial hypertension with or without left ventricular hypertrophy, diabetes mellitus, hypercholesterolemia, syndrome X, hypertrophic cardiomyopathy. In these diseases, noninvasive transthoracic Doppler echocardiography allows for serial CFR evaluations to explore the effect of various pharmacological therapies.

New noninvasive method for coronary flow reserve assessment: contrast-enhanced transthoracic second harmonic echo Doppler.
BACKGROUND We tested the hypothesis that blood flow velocity could be recorded in the left anterior descending coronary artery (LAD) during transthoracic echocardiography by use of second harmonic echo Doppler modality along with contrast enhancement (intravenous Levovist) at rest and after pharmacologically induced maximal vasodilation to assess coronary flow reserve (CFR) with a totally nonin...
متن کاملCoronary artery stenosis in asymptomatic child after arterial switch operation: detection by transthoracic colour-flow doppler echocardiography.
UNLABELLED Arterial switch operation (ASO) has become the definitive anatomical correction for transposition of great arteries (TGA). Left coronary artery (LCA) ostial stenosis was detected by transthoracic Doppler echocardiography (TTDE) as a flame like colour flow diastolic signal and coronary flow reserve (CFR) was low, 1.3. It was treated successfully by a drug-eluted stent. These findings ...
متن کاملAssessment of internal mammary artery and saphenous vein graft patency and flow reserve using transthoracic Doppler echocardiography.
OBJECTIVE To investigate transthoracic Doppler echocardiography in the identification of coronary artery bypass graft (CABG) flow for assessing graft patency. DESIGN The initial study group comprised 45 consecutive patients with previous CABG undergoing elective cardiac catheterisation for recurrent ischaemia. The Doppler variables best correlated with angiographic graft patency were then tes...
متن کاملNoninvasive assessment of significant left anterior descending coronary artery stenosis by coronary flow velocity reserve with transthoracic color Doppler echocardiography.
BACKGROUND Coronary flow reserve has been considered an important diagnostic index of the functional significance of coronary artery stenosis. With Doppler technique, it has been assessed as the ratio of hyperemic to basal coronary flow velocity (coronary flow velocity reserve [CFVR]) by invasive or semiinvasive methods with a Doppler catheter, a Doppler guide wire, and a transesophageal Dopple...
متن کاملPhysiologic Assessment of Coronary Artery Stenosis by Coronary Flow Reserve Measurements With Transthoracic Doppler Echocardiography: Comparison With Exercise Thallium-201 Single-Photon Emission Computed Tomography
OBJECTIVES We evaluated the value of coronary flow reserve (CFR), as determined by transthoracic Doppler echocardiography (TTDE), for physiologic assessment of coronary artery stenosis severity, and we compared TTDE measurements with those obtained by exercise thallium201 (Tl-201) single-photon emission computed tomography (SPECT). BACKGROUND Coronary flow reserve measurements by TTDE have been...
